Terms & Conditions - My Diagno Card
(A Product of Shri Bharg Healthcare Private Limited)
By purchasing, accessing, using, sharing, or availing services through My Diagno Card ("MDC"), the user acknowledges and agrees to the following Terms & Conditions.
1. General Terms
- My Diagno Card facilitates healthcare discount benefits at partnered diagnostic centers across applicable cities.
- Users may receive discounts of up to 30% on eligible diagnostic tests, subject to the policies, pricing, availability, and terms of the respective diagnostic center.
- Discount percentages may vary depending upon the type of test, diagnostic center, city, branch, package, ongoing promotional schemes, or other operational factors.
- My Diagno Card is a promotional healthcare benefit platform and does not itself conduct diagnostic tests or medical services.
- The card may be shared and used by family members, relatives, friends, or known associates unless otherwise restricted by the Company.
2. Booking Process & Discount Eligibility
- To avail applicable discounts, users must contact the official helpline number or authorized booking channel prior to visiting the diagnostic center.
- Users are advised to contact the helpline at least 30 minutes before the proposed visit time.
- After successful booking confirmation, a booking reference ID may be issued, which must be presented at the diagnostic center to claim applicable discounts.
- Discounts shall not apply where users directly visit the diagnostic center without prior booking through authorized MDC channels.
- Final billing and payment shall be made directly to the diagnostic center.
- Availability of discounts is subject to successful booking confirmation and acceptance by the partnered diagnostic center.
3. Validity & Usage
- My Diagno Card is intended to provide long-term usage benefits; however, validity, services, offers, and associated benefits remain subject to operational continuity and Company policy.
- The Company reserves the right to modify, suspend, discontinue, restrict, or revise any feature, benefit, partnership, or service without prior notice.
- Unless otherwise specifically restricted, there is presently no fixed cap on the number of times eligible discounts may be availed.
4. Coverage, Availability & Limitations
- Discounts may apply to eligible diagnostic investigations including blood tests, pathology services, radiology services, imaging tests, and related investigations.
- Certain tests, packages, health checkups, outsourced investigations, specialized tests, emergency services, NABL-specific packages, consumables, home collection charges, consultation fees, medicines, procedures, taxes, or additional services may be excluded from discount benefits.
- Home sample collection services are subject to availability and the policies of the respective diagnostic center.
- Diagnostic centers reserve the right to accept or decline bookings based on operational capacity, timing, technical limitations, or internal policies.
5. Institutional & Corporate Partnerships
- Educational institutions, corporations, NGOs, organizations, associations, and other entities may collaborate with My Diagno Card for healthcare benefit programs.
- Customized cards, branding materials, mock-ups, digital creatives, sample layouts, or proposed card designs may be prepared, displayed, or shared for representational, marketing, presentation, approval, or reference purposes.
- The display of any institution's name, logo, branding, card image, mock-up, or related material on the website, promotional content, presentations, or social media platforms shall not be construed as conclusive evidence of implementation, onboarding scale, active deployment, issuance, distribution, or usage of cards among all or any students, staff members, employees, affiliates, or associated persons of such institution or organization.
- The Company reserves the right to display the names and logos of associated, existing, previous, proposed, or collaborating institutions for promotional and business reference purposes unless otherwise agreed in writing.
6. Cancellation, Rescheduling & Refunds
Users may request rescheduling by contacting the helpline prior to their scheduled appointment time.
Refunds, cancellations, delays, reporting timelines, sample rejection, or other service-related matters shall be governed by the policies of the respective diagnostic center.
My Diagno Card does not directly process or control diagnostic center refunds unless specifically stated otherwise.
7. User Conduct & Misuse
Users shall not misuse booking IDs, impersonate others, manipulate pricing, provide false information, or engage in fraudulent activity.
The Company reserves the right to suspend or terminate access to services in cases of suspected misuse, abuse, misrepresentation, fraudulent conduct, or violation of these Terms & Conditions.
8. Liability & Disclaimer
- My Diagno Card acts solely as a facilitator for healthcare discount access and shall not be held liable for medical negligence, quality of testing, reporting accuracy, treatment outcomes, delays, technical issues, service deficiencies, pricing disputes, or customer experience issues arising from the diagnostic center.
- Any disputes relating to tests, reports, diagnosis, medical advice, billing, service quality, or customer handling must be resolved directly with the respective diagnostic center.
- The Company shall not be liable for interruption of services caused by network failure, technical issues, server downtime, WhatsApp delays, telecommunication failure, force majeure events, government actions, pandemics, natural disasters, or circumstances beyond reasonable control.
- The Company makes no guarantee regarding uninterrupted availability of discounts, partner continuity, or identical pricing across all locations.
9. Intellectual Property & Branding
- All trademarks, trade names, logos, branding elements, card designs, creatives, layouts, promotional materials, and website content associated with My Diagno Card remain the intellectual property of the Company unless otherwise stated.
- Unauthorized copying, reproduction, distribution, modification, or commercial use of Company branding or materials is prohibited.
10. Amendments
- The Company reserves the right to amend, revise, update, or modify these Terms & Conditions at any time without prior notice.
- Continued usage of My Diagno Card after such modifications shall constitute acceptance of the revised Terms & Conditions.
11. Governing Law & Jurisdiction
- These Terms & Conditions shall be governed and interpreted in accordance with the laws of India.
- Any disputes arising in relation to My Diagno Card shall be subject to the exclusive jurisdiction of the competent courts located in :contentReference[oaicite:0]{index=0}, Uttar Pradesh.
12. Customer Support
- For assistance, bookings, institutional partnerships, or support queries, users may contact the official helpline number or authorized communication channels of My Diagno Card.
- By using My Diagno Card, the user confirms that they have read, understood, and agreed to these Terms & Conditions.
