By using the Services, You represent and warrant that You have the right, authority, and capacity to enter into a contract with the Company in respect of the Services being offered by the Company and to abide by all of the terms and conditions set forth herein.In particular:-

(ii) You are 18 years of age or above and have the legal capacity to enter into a contract to form the binding contract in your jurisdiction
(iii) You represent and warrant to the Company that you will use the website in a manner consistent with all the applicable rules and regulations
3.1 To avail the Services, You will be required to register on the Website by following the registration process as mentioned hereinbelow:
(I) Create Your Account by following the valid step.
(ii) Fill Your basic information such as name, age, sex, e-mail address, billing `address, collection address, zip/postal code, and phone number. You warrant that all information You provide about yourself or anyone else shall be true and accurate. In case, any of the information provided by you is found to be false or inaccurate, the Company may in its sole discretion, block/remove your account.
3.2 On completion of the registration process, you will become entitled to avail the Services being offered by the Company, subject to payment of the Fee.
4.1 That the term "Services" means and includes the following diagnostic tests/health packages provided by the Company at its most advanced state-of-art testing facilities.
Comprehensive Health Packages comprising a combination of two or more of the above-referred tests.
Any other test(s)/health package(s) provided by the Company
4.2 That the services are non-transferable i.e. only the card holder and its member whose name in the test/health package(s) has been assigned at the time of booking will be eligible to avail the services offered by the first party/company.
4.5 That the card holderare required to carry a photo-identification card and Customer ID at the time of the visit to the testing facility or at the time of availing Home Service.
4.6That the card holder will be required to submit other officially valid document (OVD) / deemed to be OVD's or such other documents as may be requested by SBHPL, as part of KYC process.
4.7 That the card holderhis/her express consent to SBHPL for verifying any or all the details/documents so provided.
4.8. That the card holder shall complete KYC process in full within the prescribed time and in case if I fail to complete the same for any reason whatsoever SBHPL shall have sole and absolute discretion to reject my application(s) and/or discontinue the service(s)/product(s) for which such KYC is mandated.
5. That, SBHPL does not guarantee continuous accessibility of the applications and shall not be held liable or responsible in any manner whatsoever in respect of any loss, damage that may be incurred by me due to any reasons (including but not limited to technical, systematic or server errors/issues, or any other issue occurred while undertaking KYC process and/or for reasons which are beyond reasonable control of the SBHPL).
Health Existing Member Identification Cardholders may also called SBHPL Customer Care for any grievances/queries related to Health Existing Member IdentificationCard atEmail/Customer care number.
This is not a toll-free number and normal call charges will be applicable.
1.That the Health Existing Member Identification Card is an Existing Membership Identification number used for authentication and is neither a credit card nor a stored value card.
2.That the Health Existing Member Identification Card can be used for health check-up only at limited Life Care Counters (LCF) including Multi Specialty Hospitals (MSH), Non-MSH and diagnostic centres and such other counters for eligible treatments as may be decided by SBHPL at its sole discretion("Selected BusinessAssociates"). Updated list of Selected Business Associates, eligible treatments and benefits can be accessed through www.mydiagnocard.com
3. That the SBHPL may at any time, at its full discretion and without assigning any reason, decide to withdraw the Health Existing Member Identification Card service and/ or terminate any specific Health Existing Member Identification Card. Further SBHPL will have no liability to the Health Existing Member Identification Cardholder or any other person in this regard in any manner whatseover. In the event of termination of Health Existing Member Identification Card/ withdrawal of Health Existing Member Identification Card services any facility provided with Health Existing Member Identification Card services shall be immediately terminated.
4.That the Selected Business Associates may run different offers for Health Existing Member Identification Card Customers. Such offers shall be subject to specific terms and conditions of such offers, which shall be in addition to the terms and conditions mentioned herein. Accordingly, SBHPL shall not be responsible for the quality or any assurances and/or representations made in relation to such products/services/offers provided by third parties. Further, SHPL shall not be liable for any delay in delivery or no-delivery of product and Services and/or with respect to the quantity, quality, conditions, fitness, suitability or otherwise whatsoever of the product/services.
5. That I acknowledge that, Health Existing Member Identification Card is NOT a payment instrument and NOT assignable to any person. I hold myselfresponsible for any use/misuse of the Health Existing Member Identification Card, and I shall be jointly and severally responsible for any liability that is incurred through such usage.
6.That the in the event of an unauthorized access to or use of Health Existing Member Identification Card, Health Existing Member Identification Cardholder shall immediately inform SBHPL and call or write to SBHPL's Customer and request for blocking the Health Existing Member Identification Card.

8. That the Validity of Cashless packages will be considered from date of purchase and will be valid for a period of 1 year (365 Days). On expiry of period the cashless facilities will be blocked only the discount services of the card at listed Diagnostic Centres and Hospitals will be valid.
9. That the membership fee is as per memo shared and it will at sole discretion of company to change the same as and when required without and prior intimation.
10.That the duplication and copying of Health card is a criminal offence and such activity or fraud will take under serious crime and appropriate legal action will be taken against the person.
11. That the Card can solely used on Diagnostic Centres and Hospitals referred as associated enterprises as per annexure and no other usage is allowed, further company is not responsible for any such usage apart from associated labs.
12. That in case of loss of card, it is responsibility of card holder to intimate the company and make an application for issue of duplicate card, company can issue a duplicate card after receiving the requisite fee as decided by company.
13.That the all the condition and facilities are valid for up to validity of card, after expiry all the terms and conditions will automatically will be forfeited.
14. That the card holder has right to use the card at any associated labs and hospitalshe/she can choose asper his/her preference.
15.That the card holder should make payment of fees as decided by company in designated account of company and no service charges will required to be paid to agent.
16.That it is at the sole discretion of company to associate or terminate the agreement with the associated Labs, and company will be responsible for only those centres that are active at time, by no means company will be responsible for centres that are closed or with contract is terminated.
17. That the Company is responsible for cashless tests and discount on total bill under discounted tests. Any other facility like home sample collection, ambulance or any other charges over and above testing charges will be borne by card holder and will be paid directly to diagnostic centre.
18. That the customers/card holders will be provided with cashless test facilities/discounted test facilities in home sample collection also by the sample collection boy.
19. That the customers will have to get their “MY DIAGNO ID” verified at the hospital during the admission process of the patient for the I.P.D to get reimbursement from the company on IPD total bill.
20. That the customer should fill the reimbursement form provided on the website and follow the correct procedure to get reimbursement.
21. That the customer can not avail other insurance policies simultaneously using “MYDIAGNOCARD”.
22.That the customer must submit the reimbursement within 30 days of discharge from the hospital after that no reimbursement will be given.
23. That the company will not be responsible for any errors from the end at the process of verification in Diagnostic centres/ hospitals.
24. Company will not be responsible for any unavoidable issues caused by diagnostic centres or hospitals.
